Intake Process

To ensure our team can provide you with the highest level of care, all new patients are required to complete our intake process. This helps ensure that both you and our team have a clear understanding of expectations and care goals. Please follow the steps below:

  1. Call our office at 715-861-2300. During this call, one of our front desk team members will discuss pricing with you, create your patient profile, and send you the necessary intake paperwork.
  2. Complete and return your intake forms. After your initial call, you will receive a welcome letter and intake forms via email. Please complete these forms and return them to Spero Wellness Clinic as soon as possible, either by email or mail.
  3. Submit all required paperwork before scheduling. We are unable to schedule your intake appointment until all required forms have been completed, signed, and returned. This ensures mutual understanding regarding patient care, clinic policies, and confidentiality.
  4. Schedule your Functional Medicine intake appointment. Once we have received and reviewed your completed paperwork, a member of our team will contact you to schedule your appointment. At that time, you will be scheduled with either Dr. Endrud or our Nurse Practitioner, Kylee Anglemyer. Patients wishing to see Dr. Gering must first have their intake case reviewed and approved by her, as she has elected to focus on more complex cases and cancer patient care.
  5. A non-refundable $400 deposit is required at the time of scheduling.


Intake Appointment Cancellation Policy

Functional Medicine intake appointments are reserved for 120–180 minutes to allow for comprehensive care. If you need to cancel or reschedule your appointment, we ask that you provide at least 10 business days' notice by calling our office at 715-861-2300.

  1. Please note that deposits for Functional Medicine intake appointments may be forfeited if an appointment is canceled with less than 10 business days' notice. We understand that unexpected circumstances can arise and will consider extenuating situations on a case-by-case basis.
  2. In the result of a No-Show to an intake visit the entire deposit fee will be forfeited. To reschedule, we will then require a non-refundable deposit for the entire visit. If your visit does not take the full allotted time, then the remaining balance will remain as a credit on your account to be used towards supplements, services, or future visits.


Intake Prices

We require all new patients to submit a $400 deposit when scheduling their initial intake appointment. Please refer to our Pricing page for accepted payment methods and appointment fee rates.


Following your visit, full payment is due for the time spent with your provider. Because appointment costs are based on the length of the visit, the total fee for an initial appointment typically ranges from $800 to $1,200.


If cost is a concern, please let us know. We are happy to work with you to make your appointment more accessible, whether that means keeping the visit shorter or helping you take advantage of financing options through Cherry, our payment plan partner.


What to Expect

Once your appointment has been scheduled, you will receive appointment reminders leading up to your intake visit. We kindly ask that you arrive 10–15 minutes early to complete registration and any remaining pre-appointment forms.


After you have checked in with our front desk team, a member of our healthcare team will prepare you to meet with your provider. During your appointment, you will review your health history, discuss your goals, and address any concerns so that your provider can work with you to develop a personalized plan to help you achieve better health and well-being. While we may not have all the answers, we are committed to supporting and guiding you on your health journey.



Following your appointment, you will be offered a complimentary health coaching consultation with Julie. Together, you will create a practical plan of action and address any remaining questions you may have from your initial appointment.
